Life Solutions Counseling Services medical social workers work with home health agencies and hospice agencies to conduct admission and provide home visits to their patients. We conduct needs assessment and provide psychosocial support to individual patients, caregivers, families, or vulnerable populations so they can cope with chronic, acute, or terminal illnesses, such as Alzheimer's disease, cancer, or AIDS.
We advise family caregivers, counsel patients, and help plan for patients' needs during admission and after discharge. We also arrange for at-home services, such as meals-on-wheels or home care and sits in interdisciplinary teams to discuss the whole needs of each patient.
